Neurodegenerative diseases characteristically develop insidiously and affect specific neural structure or specific neural function predominantly. Usually, there are no biological or radiological markers to diagnose the diseases early in their course. However, eye movement abnormalities could reflect subtle pathologic process in the brain. Therefore, it is not surprising that eye movement abnormalities could be a possible surrogate marker for neurodegenerative diseases. Moreover, it could also reveal the amount of pathologic burden. Recently, the role of basal ganglia on saccadic eye movement has been elucidated more clearly. Herein, we describe clnical importances of eye movement abnormalities in neurodegenerative diseases, especially which affect the basal ganglia mainly.
